Privacy Practices

How your health information is used and protected, and the rights you hold — in plain language.

Effective July 31, 2026. This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

How we may use and disclose your health information

Treatment. We use your health information to provide your care and may share it with other clinicians involved in that care — for example, a specialist we refer you to or a pharmacy filling your prescription.

Payment. The clinic is direct-pay, so we bill you rather than an insurer. If you request a superbill to seek reimbursement, we prepare it for you to submit to your insurance company.

Health care operations. We may use your information to run the practice — quality review, scheduling, and internal record-keeping — always limited to what the task requires.

Other uses. We may contact you about appointments and, only if you have opted in, send occasional clinic emails you can leave at any time. Uses beyond these — including most uses for marketing, and any sale of your information — happen only with your written authorization, which you may revoke.

Your rights

You have the right to see and receive a copy of your record; to request corrections; to request restrictions on certain uses; to ask that we contact you in a specific way or place; to receive a list of certain disclosures; to receive a paper copy of this notice; and to be notified if a breach compromises your information. To exercise any of these rights, contact the clinic.

Our duties

We are required by law to maintain the privacy and security of your health information, to give you this notice of our duties and practices, and to abide by the notice currently in effect. If we materially change it, the updated notice will be posted here and available at the clinic.

If you believe your privacy rights have been violated, you may file a complaint with the clinic or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will never face retaliation for filing a complaint.
