What to include

Help me be ready

This quick request asks for a few things that let me prepare before you come in:

  • What the visit is for — your main concern, in a sentence or two, and how long it's been going on.
  • What you've noticed with it — related symptoms, and anything that makes it better or worse.
  • What's changed — any new diagnoses, or medications and supplements you've started or stopped.
  • Refills you'll need, and whether you'd prefer in-person or telehealth.
