After-hours visits carry a premium that reflects what it takes to be available: $400 per hour when scheduled ahead, and $500 per hour for an unscheduled urgent home visit — the kind of situation that is truly pressing but doesn't belong in an emergency room. For home visits, drive time to and from the office is billed at $255 per hour, and visits are subject to availability.
